Chronic Tick-Borne & Vector-Borne Disease Consultation

For people with known, suspected, or previously treated chronic tick-borne/vector-borne illness, or persistent relapsing symptoms where Lyme disease, Bartonella, Babesia, tick-borne relapsing fever Borrelia, Anaplasma, Ehrlichia, Rickettsia, or other vector-borne infections may be contributing.

Not appropriate for emergency symptoms such as chest pain, fainting, severe headache, neck stiffness, facial weakness, shortness of breath, irregular heartbeat, high fever, severe neurologic symptoms, or rapidly worsening illness. These symptoms may require urgent in-person care or emergency evaluation

This option is for patients without insurance, patients whose plans are not accepted, or patients who prefer not to use insurance.

The full visit fee of $600 for a new patient visit and will be charged at the time of scheduling. Return patient visits are billed at $400/hour

If you would like to use insurance and believe Dr. Jade Stefano participates with your plan, please use the insurance booking option instead.



Accepted plans currently include Premera Blue Cross, LifeWise Health Plan of Washington, and Kaiser PPO (First Choice Health Network). Many out-of-state Blue Cross Blue Shield plans may also be eligible through the BlueCard® program.

Before scheduling, please verify that Dr. Jade Stefano participates with your insurance plan. If we do not participate with your plan, please use the Cash-Pay booking option.

A credit card on file is required to schedule your appointment.

An estimated patient responsibility of $150 will be charged upon appointment confirmation. This estimate will be applied toward your deductible, copayment, and/or coinsurance. Actual patient responsibility will be determined after insurance claim processing. Any overpayment will be refunded and any remaining balance billed.

Insurance eligibility and benefits will be reviewed when possible prior to your visit. Verification of benefits is not a guarantee of payment.

If your insurance plan is not accepted, insurance information is not provided, or eligibility cannot be confirmed, we reserve the right to convert your appointment to a self-pay visit.
